About Evoke, Cost, and Membership
Evoke is a telemedicine-only practice, so all visits happen online from wherever you're comfortable. No driving across town or sitting in a waiting room. I'm currently licensed to see patients who live in Missouri, Arkansas, Kansas, Iowa, and Colorado. If you're in one of those states, you're good to go.
You will be seeing me, Samantha Neville, a board-certified nurse practitioner with over 20 years of experience. Evoke is a single-provider practice, so you'll work directly with me every step of the way. I keep the practice small on purpose so I can keep costs reasonable for people who want a functional medicine approach without the price tag that usually comes with it.
I don't just focus on one thing. A lot of hormone clinics check your testosterone or estrogen, write a prescription, and send you on your way. Weight loss clinics get you started on a medication and never really look at what's driving the weight gain in the first place. That's not how I practice.
I'm a primary care provider with a functional medicine approach, which means I'm looking at your whole body, not just one symptom. Hormones, weight, energy, gut health, and mood don't work in isolation. Your thyroid, blood sugar, cortisol, nutrient levels, inflammation, and gut all affect whether any treatment works the way it should.
That's why I recommend a more complete panel upfront. It's not about running extra labs for the sake of it. It's about catching the hidden issues that keep people stuck. I've seen too many patients spend months on a treatment plan wondering why they still feel terrible, only to find out their vitamin D was tanked, their iron was depleted, their thyroid wasn't converting well, or their insulin was quietly working against them the whole time. I'd rather find all of that on day one so we can build a plan that actually gets you feeling better.
No. I'm a licensed primary care provider and I address a lot of primary care concerns, but Evoke isn't set up to be your designated PCP. You'll still want a PCP for routine screenings like mammograms, pap smears, and colonoscopies, and please let them know about anything I prescribe.
Evoke also isn't an emergency service. For anything urgent, go to urgent care or the ER, or call 911.
Most new patients start with the $75/month Evoke membership, which covers hormone support, weight management, messaging access, and full serum lab reviews.
There's a 2-month minimum, and you'll also need the initial lab panel. Based on current lab pricing, your first two months should be about:

Lab prices are set by Labcorp and can change without much notice. I update this page whenever a panel price changes by more than $10, so your cost should be close to what's listed here. If you'd like an exact breakdown before your labs are drawn, just ask and I'll send it over.
Your membership doesn't start until we either order your labs or meet and start working together. You won't be paying for time you're not using.
Prescriptions tied to your membership will stop at the end of your active period, so you'll want to have another provider lined up if you need ongoing medication management.If you'd like to keep prescription access at a lower cost, I offer a $35/month RX-only membership. This covers the ongoing oversight, refill management, dose adjustments, and documentation I'm legally required to provide when prescribing.
Can I still order supplements after I cancel?
Yes! Your Fullscript supplement access stays open, so you can keep ordering whenever you need.
Your first visit and detailed lab review take a lot of time and planning on my end. I'm digging through your history, your symptoms, and every lab value to build a plan that's specific to you. Two months gives us enough time to get that plan going and start seeing how your body responds.
After your first two months, your membership continues month to month. No long-term contract.If you decide to cancel, I just ask for 30 days notice. Billing runs automatically, and since I'm a one-person practice, catching a cancellation the night before (or over a weekend) before the charge processes is nearly impossible. Refunds also come at a cost to the clinic, so the 30-day notice keeps things simple and fair for both of us. Your membership stays active during that 30 days, so you still have full access to messaging and care.If the timing ever creates a financial strain, just reach out. I'm always willing to work with you.
I don't bill insurance. Evoke is a membership-based practice, which keeps things simple and lets us focus on what you actually need instead of what insurance will approve. You can still use your insurance for things like imaging and medications when it makes sense.
You can also use HSA or FSA funds for your membership, supplements, and prescriptions through the clinic. Most patients keep their HSA card on file and it runs just like any other card for monthly payments.
Getting Started and Ongoing Care
A lot of patients have told me they weren't sure what came next after signing up, so here's the whole process from start to finish.
1. Sign up online
Start by signing up on the website. This creates your chart so I can learn a little about you.
2. I review your chart
I personally look over every new chart. Once you're approved, you'll get an email letting you know.
3. Sign your new patient documentsn
After approval, you'll receive four separate emails: HIPAA, Consent, Clinical Policies and New Patient Intake Form. Please sign all three. If you don't see them, check your spam or promotions folder because it is sent through the medical records system and gets flagged as spam a lot of times.
4. Complete your new patient intake form
This is where you tell me your story: your symptoms, history, medications, supplements, and what you're hoping to get out of working together. The more detail you give me here, the more I can do with it. If you already know you want any lab add-ons, let me know on this form so everything can be done in one draw.
5. I send your lab requisition
Once I review your intake, I'll email your lab requisition along with detailed instructions on how to prepare and when to get them drawn. Please read all of it so we don't have to repeat any labs. Your membership starts once your labs are ordered.
6. Get your labs drawn at Labcorp
Labs need to be done before your first visit so we have real data to work with. Bring your requisition (a copy on your phone works too). Right after your draw, email me the details I ask for in your lab instructions, like your supplements, cycle day, and hormone dose timing.
7. Set up your Patient Hub and schedule your first visit
Schedule your initial visit 8 to 10 days after your lab draw so your results are back and reviewed in time. To schedule, you'll need your Patient Hub. You can't sign up on your own. I'll email you an invite link after you're approved, and you'll use that link to create your account. Use the same email that's on your chart so everything links up correctly. I send every invite manually, so if you haven't received yours by the time your labs are drawn, check your spam folder, then text me and I'll send it right over.In the hub you can:
The hub doesn't allow cancellations, so if you need to change a visit, book the new time and text me to remove the old one.
8. Your initial visit
We'll go over your labs together, talk through what's going on, and build a plan that's specific to you.
After your first visit, most of our communication happens through email or text. This is the easiest way to ask questions, give updates, or let me know how you're feeling on a new treatment.Visits are usually saved for:
If you feel like something isn't getting resolved over email or text, then feel free to schedule a visit through your Patient Hub at anytime. Sometimes it's just easier to talk it through on a phone call.
Lab Specific Questions
Yes, if they're recent and complete enough. If you've had a functional medicine workup within the last 3 months, note which tests you've had next to the required panel on your New Patient Intake Form and attach your results. Anything that hasn't been done will still need to be drawn.
Why 3 months? Things like vitamin D, ferritin, thyroid, and insulin can change quite a bit in that time, especially if you've started new supplements or medications since. Older labs may not reflect where your body is right now.
Hormone labs have to be drawn at the right time to count. For women with a regular cycle, that means day 19 to 21. For anyone on testosterone injections, it means the morning before your next injection. If your previous hormone labs were drawn at a different point, I'll likely need to repeat them so I'm reading them accurately.
Basic labs from a routine physical usually don't cover the full panel, so most patients still need at least some of it drawn.
Timing matters a lot for hormone labs, because your levels change throughout your cycle and throughout the day after a dose. Drawing at the right time is what lets me read your results accurately, so we don't have to repeat them.
Everyone: ALWAYS Go early in the morning, ideally between 7 and 9 am, fasting with water only after midnight.
Before your labs can be ordered, I need to approve your enrollment, you'll sign your documents, and I'll review your intake form. So give yourself about 2 weeks before your day 19 to 21 window.
Here's an easy way to plan it: if your period started recently, now is a great time to sign up. Put the date your last period started on your intake form, and I'll know when your window is coming up.
If you miss the window, that's okay. Just wait for your next cycle and go on day 19 to 21 then. Keep in mind that if labs aren't drawn within the billing cycle they're ordered in, the lab charge still processes on the 1st of the following month.
If hormones aren't your main concern, let me know. If something else is more pressing, like fatigue, thyroid, blood sugar, or nutrient levels, we can split the panel. That way you can get those labs drawn now and do the hormone labs on the right cycle day. Splitting the panel adds a $3.50 draw fee for the second visit.
Your labs are billed through Evoke as "Client Bill" at wholesale cost, which is much lower than what you'd pay at the lab directly. Don't pay at Labcorp or Quest. If they ask for payment at check-in, just let them know your labs are "client billed" through Evoke.
When I order your labs, a charge is created in my system. I hold it and run it on your card on file once your results come back, so you're only charged after your labs are done.
If something delays your draw, like waiting for the right cycle day, just text or email me. If labs aren't drawn within the billing cycle they're ordered in, the charge may process automatically, so a quick heads-up lets me pause it.
Most patients ask to use insurance for labs at first, because that's what they're used to. Here's why I don't offer that option.
Insurance creates a frustrating loop. Insurance usually only approves labs tied to an existing diagnosis code. But you often can't get the diagnosis without testing first, and insurance won't approve the test until the diagnosis exists. Patients get stuck, and many go undiagnosed or wait months for answers.
Results get lost. Insurance-based orders don't connect to my medical records system, so I don't automatically receive the results. Tests sometimes get left off when lab staff enter orders by hand. Patients can wait weeks thinking I'm reviewing their labs when I haven't seen them at all.
Coverage is limited and unpredictable. Insurance tends to cover the bare minimum, not the deeper markers, ratios, and timing-based labs that actually explain symptoms like fatigue, anxiety, bloating, thyroid issues, or weight loss resistance. Insurance may approve half an order and deny the rest, and you won't know until a surprise bill shows up 1 to 2 months later. Sometimes claims get denied simply because I'm not in your network.
It takes time away from patient care. I've spent hours building diagnosis code lists for specialty labs, only to have insurance reject them and make me start over. As a single provider, that time comes straight out of patient care. Offering insurance billing would mean raising my prices to cover those lost hours, which is exactly why I left insurance-based medicine.
You pay what I pay. Over the last 14 years, I've worked directly with labs, compounding pharmacies, and procedure facilities to secure wholesale pricing. I don't mark up labs like many functional medicine clinics do. You pay the same price the clinic pays.
What about basic labs? You can still ask your PCP or specialist for basic labs insurance usually covers. Most won't order hormone or specialty panels, though, because they expect them to be denied. And splitting labs across two systems delays your results reaching me, which slows down the full picture.
For most people, self-pay ends up being simpler, faster, cheaper, and far less frustrating. Your results come straight to me, and we can look at your symptoms, hormones, nutrients, gut health, and metabolism together as one full picture, based on how your body actually works, not on insurance rules.
